How to Access your Medical Record?
You can now view your medical record online (at the present time this only includes Medical problems and Laboratory results).
If you would like to request access you must complete the form below or alternatively you can collect one from the surgery.
When you have read and completed the form you need to hand it in to reception at the surgery with your ID (ID options are listed on the form). The surgery will then contact you within 14 days to notify you that your records are ready to access.
